Getting Started:

• To begin, click the link below to request a free, 15-minute, virtual appointment. 

• If it is determined that you are a good fit for Sylvie's practice then you will complete intake forms through the online portal.

• After completing the consent and intake forms you will schedule a 60-90 min initial consultation.

• Note that, following the initial consultation, all addiction services are offered in person unless you are sick or in an emergency. 

• With Ohio regulations, we can offer virtual options—no in-person sessions are required.

• Therapy and medication management (CBT, Motivational interview, Trauma-focus)

Costs:

• $175 for the Initial consultation - 60-90 min

• $125 for each additional session (20- 30 minutes)

• Labs and prescriptions not included. You can use insurance for prescriptions and labs.
